Dear Parents/Guardians,
Flu Vaccine Clinic in school tomorrow Friday October 15th for the pupils of our school. If you want your child(ren) to receive the flu vaccine we ask you to do the following:
1) Read the information pack leaflet here: Flu Vaccine leaflet for parents.pdf
2) Fill in the consent online form on www.localpharmacy.ie > Book a vaccination > Schools > Your School. If you do not have access to the internet, then fill in the form on the second page and return to your school before the deadline. This form is available here: Childrens Consent Form.pdf
3) Due to Covid regulations, parents aren't permitted to the vaccination clinic.
NOTE: This is a nasal spray.
